Liability
Paws ‘n Pads Pet Care employs extensive safety and sanitary precautions to ensure your dog(s) safety and health. We take care of your dog(s) as one of our own family members.
Due to our open run environment, despite reasonable and prudent efforts, certain activities that dog(s) may participate in, including group play and one-on-one play with other dog(s), could result in injuries or illness. Illness may occur from exposure to parasites, viruses, and other medical conditions passed from dog-to-dog. Injuries or ailments may include sprains, strains, scratches, bites, broken bones, fatigue, dehydration, nicks, cuts, or death. Not each and every potential risk can be listed, but nonetheless the client agrees that the benefits associated with dog socialization outweigh the possible risks.
By signing this agreement, you (the client) agree to relieve Paws ‘n Pads Pet Care, and it’s owners and staff from any and all liabilities, claims, demands, actions, or rights of action, which are related to, arise out of, or are in any way connected with the dog’s participation in activities at Paw’s ‘n Pads.
Vaccination Records
My dog(s) is up to date on all vaccinations required by Paws ‘n Pads Pet Care, as I understand this is important for the health and wellbeing of my dog(s) and the dogs they interact with. This includes: DPP (Distemper, Parvovirus, Parainfluenza), Bordetella (Kennel Cough), Rabies, Canine Influenza (H3N2 and H3N8), negative fecal test, a monthly flea and tick preventative, and regularly trimmed nails, as well as being spayed or neutered (or will be when they are of age/size). I agree that I will provide a copy of my dog(s) up to date vaccination records regularly when they have received their vaccinations.
Authorization of Medical Care
If my dog is ill or injured while participating in activities at Paws ‘n Pads, they will make every reasonable effort to reach me pursuant to the contact information I have provided. However, if they are unable to reach me, I consent to have Paws ‘n Pads seek appropriate veterinary care and I accept responsibility for any and all associated expenses. Paws ‘n Pads will not pay any portion of veterinary expenses associated with medical care for the dog if so necessary.
Damage
I accept the responsibility of paying for any damage to the facility, property, and/or equipment caused by my dog.
